If you are the Primary Member, enter your 4-digit member number
If you are the Spouse (or Significant Other with a Family Membership), enter your 4 digit member number and add "-1"
Enter your temporary password, which is your last name in lower case

Example for Primary Member:
Username: 0080
Password: albert

Example for Spouse:
Username: 0080-1
Password: albert
